Our Story
Heim Health was founded in 2022 by clinicians and engineers who watched the pandemic expose the critical gaps in remote care infrastructure. They saw patients discharged from hospitals without adequate follow-up, care teams overwhelmed by phone check-ins, and health systems unable to act on the signals hidden in patient-generated data.
The founding team set out to build a platform that closes those gaps — one that gives providers the right information at the right time, and gives patients tools to stay engaged in their own health between visits.
Today, Heim Health supports hundreds of care sites across the United States, helping health systems, physician groups, and home health agencies deliver better outcomes for patients living with chronic conditions.
